Communities in Charge

Background
Philosophy/Focus Communities in Charge is designed for local communities interested in improving access to care for low-income, uninsured individuals by rethinking the organization and financing of local care delivery. The initiative will help broad-based community consortia design and implement sustainable new delivery systems that manage care, promote prevention and early intervention, and integrate services.

Grants will be awarded in two stages under this program. Phase I grants are for Organization and Planning; Phase II grants are for Development and Implementation.

Up to 1,100 hours of direction and technical assistance for Communities in Charge will be provided by Medimetrix, a national health consulting firm. Medimetrix staff will meet with each community awarded a grant to help determine the scope and timing of technical assistance that will be provided.


Major Fields of Interest Access to basic health care; community projects; direct services; tobacco, alcohol, and other drug cessation.

Type of Grants Organization and planning grants, matching funds.

Grants Restrictions To be eligible, communities (i.e., cities, counties, or the equivalent) must have a minimum population of 250,000 and at least 37,500 low-income residents who are uninsured. If more than one organization from a community requests an application package, the applicant organizations will be asked to work together to submit a single application. The communities must demonstrate matching grants for Phase II.

Grants Analysis
Total Annual Giving One-year grants of up to $150,000 will be awarded to a maximum of 20 communities in Phase I. For Phase II, three-year grants of up to $700,000 will be awarded to a maximum of 15 communities that successfully complete Phase I.

Application Guidelines
Selection Criteria Foundation will not fund disease-specific organization activities unless the program is presented in a more global context. Projects must provide community-based services addressing access to basic health care and represent an innovation to the community. Priority funding will be given to projects directed toward underserved populations; involving cooperative arrangements; having multiple funders; having long-term viability once RWJ funding is completed; and having local government, public and private health care provider, and consumer action group support.

Application Procedure Interested parties must request an application package from the National Program Office to apply for a Phase I: Organization and Planning Grant. A workshop will be held to answer questions about the application and selection process. Attendance may be helpful, but is not required.
